WalletHub's latest report shows where immunization coverage lags the most across the United States.
Vaccination rates remain uneven across the country.
While some states consistently achieve high coverage, others continue to struggle, leaving residents more vulnerable to preventable disease outbreaks. WalletHub’s 2025 analysis ranked all 50 states — and Washington, D.C. — using 16 vaccination metrics, from childhood immunizations to adult flu and shingles shots.

The states with the lowest vaccination coverage typically have weaker public health infrastructure, higher uninsured populations and cultural or policy resistance to immunization.
Southern states dominate the bottom tier, with Mississippi, Alabama, and Georgia clustered at the very end of the list.
WalletHub's findings illustrate the challenges of reaching vulnerable populations in low-coverage regions. Lower vaccination rates increase the risk of outbreaks, heighten demand for acute care and place added strain on already stretched health systems.
The rankings also highlight the continuing influence of policy, insurance access and public attitudes on health outcomes.
