The good news is that patient safety improvements are achievable, as many hospitals across the country are successfully reducing harm through targeted interventions, even those without top safety grades. National data shows significant reductions in hospital-acquired infections and adverse events — sometimes as much as 40% over the past decade —demonstrating that real progress is underway.
