
CenterWell to take over shuttered Walmart Health spaces in 23 locations in four states
CenterWell says the centers aim to provide seniors with greater access to tailored primary care services
CenterWell, the health care services business of Humana Inc. announced its intent to lease clinical space and open senior-focused primary care centers at 23 Walmart Supercenter locations in Florida, Georgia, Missouri, and Texas. These centers, which will operate under the CenterWell Senior Primary Care and Conviva Care Centers brand names, aim to provide seniors with greater access to tailored primary care services, according to the company.
The initiative involves transforming former Walmart Health spaces into dedicated primary care centers specifically designed for older adults. CenterWell anticipates that these centers will be equipped, staffed, and operational by the first half of 2025.

"We are looking forward to welcoming CenterWell into these purpose-built health care spaces to offer quality care to communities in four states,” said Brian Setzer, executive vice president of Walmart Health & Wellness, in a statement. “Leasing these spaces to a well-known and successful health care delivery organization is a win for customers and patients, as we continue to focus on our core health & wellness business of pharmacy and optical."
CenterWell Senior Primary Care and Conviva Care Centers, as part of Humana's Primary Care Organization, are the largest and fastest-growing senior-focused, value-based primary care providers in the nation. As of March 31, 2024, they deliver care to approximately 318,000 seniors across nearly 300 centers in 15 states. These centers are equipped with dedicated staff, including board-certified physicians, nurse practitioners, medical assistants, care coaches, social workers, behavioral health specialists, and clinical pharmacists, all working together to meet the unique medical and social needs of seniors, according to the company.
The new primary care centers will be located adjacent to Walmart Supercenter stores in the following metropolitan areas:
- Tampa/St. Petersburg
- Orlando
- Jacksonville
- Atlanta
- Dallas/Fort Worth
- Kansas City
