A focus on fraud, waste and abuse

Federal prosecutors and regulators opened 2026 with a wave of health care fraud actions that, as of March 10, have charged or resolved cases collectively alleging more than $2.8 billion in fraudulent schemes.

The cases span virtually every corner of U.S. health care — Medicare Advantage risk adjustment, durable medical equipment (DME) billing, genetic testing labs, Affordable Care Act (ACA) marketplace enrollment, compound drug prescribing and home health — and include some of the largest individual fraud recoveries in Department of Justice (DOJ) history.
