Medical Guardian launches Guardian Flex, its smallest wearable medical alert device

Medical Guardian, a provider of connected health and safety solutions, has launched Guardian Flex, a wearable medical alert device the company describes as its smallest to date. The device is designed to combine 24/7 emergency monitoring with a more compact, adaptable form factor intended to encourage more consistent daily wear among older adults.

Guardian Flex is approximately 30% smaller than Medical Guardian's previous-generation wearable and comes in gold, silver and black finishes. It uses a proprietary quick-release accessory system that lets users switch between wrist, pendant and belt-clip configurations without tools or device re-pairing.

"For more than two decades, Medical Guardian has been committed to helping older adults live safely and independently," said Geoff Gross, founder and CEO of Medical Guardian, in a statement. "In listening to thousands of members and caregivers, we heard a consistent message: protection should fit naturally into everyday life, not define it."

According to the company, Guardian Flex is the first device built from one of Medical Guardian's largest proprietary research efforts, drawing on input from more than 10,000 members, caregivers and prospective buyers. That research found that nearly three-quarters of respondents live alone, and more than half reported a fall within the previous year. The company said the findings pointed to demand for devices that feel less clinical and more like everyday accessories, while still supporting remote patient monitoring-style connectivity between users and their support networks.

On the technical side, Guardian Flex includes nationwide 4G LTE cellular connectivity with integrated GPS, two-way voice communication and fall-detection technology the company says was trained on real-world fall data. Medical Guardian notes that fall detection does not identify every fall and that members should still press the help button when possible. The device offers up to five days of battery life and carries an IP67 water-resistance rating.

Additional features include a "Care Circle" function that sends real-time notifications to designated family members or caregivers during emergencies, a "Ring My Device" locator feature for misplaced units, and a companion mobile app that displays battery status, signal strength and daily activity information.

Gross said the broader goal behind the device extends beyond emergency response. "The future of aging technology should not be defined by limitations, but by possibility," he said. "We believe technology can fundamentally change the experience of aging — giving people greater confidence, freedom, and independence to live life on their own terms."

A growing wearable market for aging in place

Guardian Flex arrives as device makers continue to compete on comfort and discretion as much as on core emergency-response functionality. Traditional personal emergency response systems (PERS) have historically faced adoption and adherence challenges — studies have found that a substantial share of older adults who own a medical alert device do not activate it during an actual fall, often citing device stigma or forgetting to wear it as barriers.

That has pushed manufacturers toward smaller, more discreet form factors, interchangeable wearing styles, and integration with broader connected-health ecosystems rather than standalone alert buttons. The shift mirrors a wider trend in aging-in-place technology, where vendors are increasingly pairing emergency response with passive monitoring, caregiver notification tools and mobile apps that give family members visibility into an older adult's day-to-day status without requiring active check-ins.

For physicians managing older patients, particularly those living alone or with a recent fall history, this expanding category of consumer-facing wearables represents another data point — and potential care-coordination tool — outside the traditional remote patient monitoring devices reimbursed through Medicare. As adoption grows, some practices may find these consumer alert systems increasingly relevant to conversations about home safety and independent-living risk assessment, even though they typically operate outside clinical monitoring workflows and reimbursement pathways.
