Every deadline hitting physician pay through 2028

Medicare rates, prior authorization, telehealth and Medicaid coverage all change between Sept. 14, 2026 and Jan. 1, 2028.

Physicians spent the first half of 2026 absorbing a fee schedule that finally moved in the right direction. That reverses in January. The 2.5% conversion factor increase Congress wrote into Public Law 119-21 was built to last one year, and it expires Dec. 31. What replaces it is a proposed cut, a mandatory risk model for specialists who never asked to join one, a second telehealth cliff and a set of Medicaid changes that will move patients off physician panels for paperwork reasons rather than clinical ones. Four of those changes take effect on the same morning, Jan. 1, 2027.
