Medical Economics sat down with Susan Dentzer, president and CEO of America's Physician Groups, to discuss their latest report, "Medicare Done Right."

In this clip, Dentzer explains a key recommendation from the report: ensuring every Medicare beneficiary is in an "accountable relationship" with a health care professional.

"What it really means ... is having a primary care physician — or optimally, a primary care team — that essentially constitutes an individual's medical home," Dentzer said.

She emphasized that this team should serve as both the first point of care and the hub for coordinating across a fragmented health care system. The ultimate goal? Keeping patients healthier and out of the hospital by improving care coordination, specialist referrals and chronic condition management — a model Dentzer said is achievable, even for smaller, independent practices.
