Jon Leizman, MD

Jonathan Leizman, MD, is the chief medical officer of Premise Health, where he works to enhance the delivery of high-quality, integrated health care and exceptional experiences, while advancing clinical delivery and access models. Dr. Leizman previously spent 18 years in progressive leadership positions within the Cleveland Clinic Health System and is board certified in family medicine.
