What the 2026 midterms put in play for physicians

Fact checked by: Keith A. Reynolds

Control of Congress, 30 governorships in Medicaid work-requirement states and eight statewide health care measures were on Tuesday's ballot. Here is what each one would change for physicians and their patients.

Thirty of the 44 states that must start Medicaid work requirements held governor's races Tuesday, according to KFF. The requirements take effect Jan. 1, 2027.

Every House seat was also on the ballot, along with control of a Senate that will shape Medicaid financing, Affordable Care Act (ACA) subsidies and the Medicare physician fee schedule.

Beyond the federal races, voters in seven states face statewide measures that reach into exam rooms and practice finances, including a trigger that could end South Dakota's Medicaid expansion, a one-time billionaire tax for health care, a spending floor for California community clinics, four abortion measures and a Colorado limit on surgery for minors.
