Each year, ECRI--an independent authority on health care technology and safety--and the Institute for Safe Medication Practices compile a listing of the top 10 safety issues confronting the health care industry. Staff from both organizations analyze information and evidence from a variety of sources ranging from scientific literature and trend reports to reported medication safety problems and accident investigations. Then they rank the identified issues based on their severity, likelihood, number of patients possibly affected, difficulty in recognizing or solving, and pressure the issue places on the organization.
