Where patients live shapes their health in ways that go well beyond the doctor’s office. Access to fresh food, walkable streets, affordable fitness options and well-maintained green space can determine whether a community thrives or struggles with chronic disease. The variation across American cities is significant.

WalletHub's 2026 Healthiest & Unhealthiest Cities ranking, published March 30, evaluated 182 of the largest U.S. cities across 41 health indicators organized into four equally weighted categories: health care, food, fitness and green space.

The gap between the healthiest and unhealthiest cities is stark. The No. 1 city scored 70.41 out of 100. The No. 182 city scored 24.34.

In individual metrics, the disparities are even wider. The cost of a medical visit varies threefold between the cheapest and most expensive cities, the premature death rate varies fourfold, and the ratio of mental health counselors per capita varies ninefold.
