Medicare’s new ACCESS payment model, which will combine primary care and specialty care with technology to manage patient chronic conditions, is a month out from its official start on July 5.

ACCESS is short for Advancing Chronic Care with Effective, Scalable Solutions. It is a 10-year voluntary model to help patients manage high blood pressure, diabetes, chronic musculoskeletal pain, and depression. Those conditions affect more than two-thirds of Medicare beneficiaries.
